General Information for Planners

Please take a moment and read through the Policies and Procedures section before you start the accreditation application process.

*Note that a UC San Diego School of Medicine faculty member must be a major decision maker in the planning of your activity and take responsibility for validating the content and ensuring compliance with all accreditation requirements.

If this is your first time or this is a new activity, please complete our Activity Questionnaire. You will receive a response in two business days with next steps and information regarding fees.

Educational Planning Process/Resources

Please use one of our educational planning tools to guide you through the gap analysis/needs assessment process and subsequent layers of program planning. The use of these tools will allow you to walk through the steps involved and adequately prepare for the application submission process.

Request Disclosures

Before you begin planning your education, collect information from all planners, faculty, and all others* who are in positions to control content. Ask them to provide information about ALL financial relationships with ineligible companies within the previous 24 months.

It is important to identify financial relationships prior to the activity beginning so that the relevant financial relationships can be mitigated in a manner that is appropriate to each person's role.

*Others include the activity planner/coordinator/submitters.

Disclosure collection must be done in CloudCME. Refer to the following instructions/templates for this required step in the planning process:

Prepare to Submit the Application

The following will be required to complete the application. Review and use the templates and resources below.

Completed Disclosures in CloudCME from the Planning Committing and Faculty (refer to How to Check Disclosure Status)
Program Agenda (not required for RSS)
Budget
Faculty List (not required for RSS)
Practice Gaps/Needs Assessment
List of Activity Supporters (if applicable)
Additional Documentation as needed

Create a CloudCME profile by following these instructions.

All planning members and faculty must have disclosure completed in CloudCME prior to submitting the application.
If disclosure have not yet been submitted, please use the Disclosure Follow Up Email Template.

After Submitting the Application

Upon submission, you will receive an automatic email confirming your submission.

The application will be reviewed by the CME office and feedback will be provided via email.

For applications that are found to be incomplete or not meeting standards, the CPD Office will work with the Course Director/Planning Committee to revise as needed.
